Your visit with a physician in our clinics is covered by OHIP, however, there are charges for other services such as injections, form completion, and notes. We follow the guidelines of the Ontario Medical Association for these non-insured services.
Quebec residents are required to pay for their appointment with our physicians and a receipt will be provided to be reimbursed by RAMQ.
IFHP Program members need to bring their 10-digit UCI number to be covered for the visit with the physician.

Preparing for your appointment with a Sport Medicine Physician or Physiatrist at ActiveCARE Physio
Once your appointment is scheduled, it is your responsibility to ensure that you attend your appointment. Text appointment reminders are sent from our EMR system as a courtesy but are not guaranteed.
2 business days notice is required to change or cancel your appointment. Since this time was specifically reserved for you, there will be a fee for not showing up for your appointment or for failing to provide adequate notice. Missed appointments are lost opportunities for other patients who may require more immediate assistance and thus waste valuable healthcare resources.
What can I expect at my first appointment?
Our sport medicine physicians can diagnose your problem by manipulating the joint or asking you to do certain movements to recreate the pain you are feeling. They can order any tests if necessary, and prescribe appropriate treatments and medications. Our physicians will communicate with the referring physician regarding your diagnosis and treatment plan.
Please arrive 10 minutes prior to your appointment
Bring your valid OHIP card, a list of current medications, if applicable, and a list of questions you would like to ask during your appointment. If you do not have a valid OHIP card, you will have to pay for your appointment. A receipt will be provided for reimbursement, if applicable.
Wear appropriate clothing: loose-fitting comfortable clothing for ease of movement and exposure for the area you are being seen for. For knee, leg, or hip injuries, loose athletic shorts are helpful; for shoulders, a tank top is recommended.
Our clinics have multiple practitioners and can get very busy. Please only bring one support person or interpreter to your appointment as the waiting room has limited capacity.

Our sport medicine physicians offer services that allow for accurate and safe delivery of medication to the site of pain. Depending on the location of your symptoms, ultrasound guidance may be used to ensure the accuracy of the treatment.
